Actually more like Yachiru is his Zanpakuto's spirit manifestation much like the form Zangetsu takes for Ichigo. No clue has to why we have seen her since Day 1 of meeting Kenpachi though as the other zanpakutos we don't really see any manifestation from until the story arc with Muramasa (minus Renji and Ichigo).
