What asian said, also this question was already discussed a long time ago when she was released.
If you just search the forum you will find the thread of it with same topic along with battle report.

I have Shy Nemu myself.
I also have other characters with death mods.
I know from experience in all cases, that No Death Mod ever activates if the character dies by the Zanpakuto Skill.

Really? I am 100% sure in that it works.
I use her just for that mod and it works even on zan kill.
