Evolution List

So I have been thinking how awesome it would be to compile a list of all Evolution items, and what characters potentially use said items.  Unfortunately, there is no way of knowing what items a character will use unless it is apart of their current evolution level.  So like I can see what my character needs for lvl 2 evolution, but not lvl 9 for say.  This would prove very useful to snipe/buy items out of the guild shop early on, and likewise help prioritize what items ya apply for out of guild rewards.

So what I need from everyone is to post Evolution item information that they have available.  What I need to know is:
Exact Full Partner Name
Partner's Evolution Level (meaning the items you list, are they used to upgrade the partner to Evol lvl 3, or lvl 7 etc.)
Exact Item Name and Quantity Needed (and for items that require fragments please list out the fragments, not the whole item. If a character needs a Urahara Flying Cape, list it as "5 Urahara Flying Cape Fragments)

Here is a GOOD example of what I need:

Level 8 Evolution Requires:
20 Executive Militia Set Frag
15 11th division headband frag
2 sneaker
2 belt
1 reiatsu amplifier
5 Shinigamai Zampakatu frag
5 11th division glove frags
1 jinta's baseball bat
10 Ginreigojaku frags
10 Bakkato frags
5 Uruhara's flying cape
1 reiatsu gigai
1 handguard
5 Gambler Fist
1 mod soul

If you post the info in that exact way, it will be a huge help! I will update all of the information on this Google Sheet that I published to the web for anyone to view:

Please do not include info for lower grade characters like D, C, B's.  Example would be the blue/purple characters from Tavern.  This is meant for A- and above characters.  There may be a few exemptions to this, like Rukia for example, she is the only character that is not A- that is still used well into the level 100's.

I would also be curious to know how useful this document is and/or the interest in having this information available.  If it is popular/useful I will put in the time/effort to make a comprehensive/nicely formatted document...if no one posts info or cares, then i won't bother -- so let me know your thoughts.  Thanks guys!

This is definitely a great idea.  I'll start with Sexy Yoru, Evo 8 requires:

L1: 20 Executive Militia Set Frag (yellow)
R1: 10 11th division headband frag (blue), 2 sneaker (green), 2 belt (green), 1 reiatsu amplifier (green)
L2: 5 Shinigamai Zampakatu frag (blue), 5 11th division glove frags (blue), jinta's baseball bat (white)
R2: 10 Ginreigojaku frags (purple)
L3: 10 Bakkato frags (purple)
R3: 5 Uruhara's flying cape (blue), 1 reiatsu gigai (green), 1 handguard (white)
L4: 5 Gambler Fist (blue), 1 mod soul (green)
R4: 5 11th division headband (blue)

swp panther will get their evol lvl reset when you upgrade them and you have to complete your current evol lvl to upgrade

This issue has been fixed.  Upgraded Shin Shura Ken who had 3 evo's and 1 modify. (about 4-5 days ago)
After upgrading him to Noza Ken he kept all 3 levels of evo and his first modify.
