Well, there is probably a reason why your account has been blocked/disabled. If you do not receive an answer from FB then you probably should create a new account at bleach.gogames.me and this time attach it to your email instead of facebook or any other login method.
When you log into facebook, you there can manage all the apps. You can wish to remove it there, but I'm not sure if you lose all your data and probably have to start from zero.
