Your Ulq evolution is already lv4, and what you see is the mats needed for 5 I guess the same goes for Urahara right?

I have one question, let's say i recruited Shin Grimmjow A+ caracter, then i modify him and evolve him with all the needed accessories.
After that if i upgrade him to a S caracter, will the modify and the evolution remain the same ?

Yes, they will remain the same. Everything carries over when advancing a SWP partner (as far as I know).
