that is the normal form, he broke that sometime later on in the show I think

OR it was against Kenpachi where he broke his Shikai.
Since Zangetsu repaired the zan there and returned it in Shikai form to Ichigo, maybe it couldn't revert back to normal form after that, since ya know, the zan returned to Ichigo in Shikai form as that's how it was repaired to be; the Shikai form became his fixed/normal form for the zan

The Kenny fight one makes more sense since it'd pretty much be a whole new start for Ichigo and his zan, and the form before his Shikai doesn't really exist after that moment. tbh I can't really remember him specifically breaking the normal form, so yah, Kenny fight
