[1] Damage still increases after 100%. The best way to think about it is that Ichigo's attack is boosted by a percentage of the enemy's defense. That percentage is determined by the the number of rounds that have passed. More concisely, considering the enemy's base defense D and Ichigo's base attack A1, Ichigo's attack A2 after R rounds is treated as A2 = A1 + (D * 0.25 * R).
[2]This modify's damage boost is affected by defense halos but buff and debuff icons are ignored.
[3] PvE does not include daily activities with Reiatsu in it. Events such as Ryoka, Evil Spirit, and Void are classified as "PvPE" by the developers.

Ofc i hope that ignore def is based not on the initial total def but based on the total def gained thru the entire game.

if first round enemy has 100 def and HVI has 80 attack he will do 105 dmg (def added as dmg) the next round if the def goes to 120 he will do 80+(120*0.25)=110.. which is best in my interpretation.
Before it was function of enemy def which stacks, now it is just addition based on the enemy def.
I compered him before to Muken Aizen but his 500% were function of his own attack, and HVI gains 500% attack based on enemy def (+def growth)
