Mutilate was applied before invincibility buff was.  The mutilate would of missed if Koga died from Shunsui's skill.

That's only because zanpakutos ignore invincibility.

Like Mayuri said, you need to have the invincibility buff on before Koga dies to avoid being affected by mutilate.

Invincibility is like an immunity to everything, but it does not clear things you are currently hurting from.

Your Yama got a combo from your main character. Mutilate damage does not happen on combo because of a bug, I believe, but this might just be a visual bug and it is actually still happening invisibly.
